Please do not use my name as your physician for future procedures, laboratory tests, imaging, or mammograms after December 31, 2026, even if I have previously provided a prescription or referral for such services, as I will not be able to receive those reports.

I will make your medical records available to the physician of your choice. As these records are confidential, I will require a signed HIPAA-compliant authorization before releasing them.
