Am I a good candidate for breast augmentation?

The first part of the breast augmentation process is a comprehensive consultation in our New York City office. Our office is easily accessible from Brooklyn using a cab, car or public transportation.

Dr. Naidu will get to know you and your goals for the surgery. For many of her Brooklyn patients, breast augmentation is a way to feel more feminine by getting back to their pre-pregnancy body,  improving breasts which have lost volume secondary to gravity or weight loss, or repairing the breasts following injury or illness.

If you plan to become pregnant or breastfeed in the future, Dr. Naidu will explain the impact that motherhood will have on your breasts, so that you can decide if this is the best time for breast augmentation.

During your consultation, you will need to tell Dr. Naidu about your medical history and your use of medications, nicotine and/or alcohol. Dr. Naidu will conduct a physical examination of your skin and the soft tissue of your breasts. She will measure the base width of your breasts and the amount of existing breast tissue. These measurements will determine the optimal size and location of your implants.

At the end of the consultation, Dr. Naidu let you know if you are a good candidate for breast augmentation. She will educate you about your augmentation options and answer your questions. She will describe the risks and benefits of breast augmentation surgery. Dr. Naidu wants you to make an informed decision about breast augmentation and she will never pressure you to schedule the procedure.

After meeting with Dr. Naidu, her assistant will provide you with information about the cost of surgery and when your surgery could be scheduled.

Preparing for Breast Augmentation in Brooklyn

If you decide to proceed with breast augmentation surgery, you will have a second consultation with Dr. Naidu in our office. At this appointment, Dr. Naidu will help her Brooklyn patients choose the best size, shape, volume and type of implant to create a feminine silhouette. This decision is based on the measurements of your breasts, the amount of existing breast tissue, your aesthetic goals, and your body frame.

Dr. Naidu has a large variety of sizers available try on, so you can see how you will look following the augmentation. Dr. Naidu will spend as much time with you as you need, in order to determine the best shape and size for your body frame, while respecting your desires as to your cosmetic results.

Dr. Naidu will also explain her plan for your surgery. She will let you know the length of your surgery, the location of your incisions, and what to expect during your recovery. You will also complete the necessary paperwork for the surgery. At this appointment, we will take pre-operative photographs. These photographs are an important part of your surgical planning and are considered part of your medical record. They will never be shown to other patients without your written permission.

Depending on your medical history, Dr. Naidu may advise you to discontinue certain medications prior to surgery. If you are a smoker, you must not smoke for eight weeks before or after the surgery. If you are over the age of 35 or have a family history of breast cancer, you will need to have a screening mammogram prior to your breast augmentation surgery. For your safety, Dr. Naidu may also order additional screening tests or request preoperative clearance from your regular medical doctor

Choosing the Right Implant for Your Breast Augmentation

The success of your breast augmentation surgery depends on choosing the best implant type, style, size and location for your body. Dr. Naidu will help make these crucial decisions based on the measurements of your breasts, the amount of existing breast tissue, your aesthetic goals, and your body frame.

Types of Breast Implants Available in Brooklyn

As of November 17, 2006, both silicone and saline implants are approved by the Food and Drug Administration (FDA) for use in cosmetic and reconstructive surgery of the breasts. Although silicone implants received a significant amount of negative press in the early 1990’s, numerous well-designed studies since that time have failed to show any increased risk of breast cancer or connective tissue disease. In March 2012, shaped silicone implants from Sientra became available, and in February 2013, highly-cohesive (“gummy bear”) implants were introduced by Allergan into the US market. The Ideal Implant, a saline implant with multiple silicone shells, was approved for clinical use in 2014.

Both silicone and saline implants come in different sizes, shapes, and textures. Size is determined by your tissue characteristics and the measurements of your chest wall. In general, a specific range of sizes fits best into each patient. Dr. Naidu typically uses smooth round implants for cosmetic breast augmentation in order to produce a natural shape. For some patients, textured teardrop-shaped, or anatomic, implants are a better fit.

The implant profile refers to how much projection from the breast wall the implant will give you. All three of the major manufacturers of breast implants in the United States make implants which have low, medium, and high projection. The choice of implant projection is determined by both your wishes as a patient and your anatomy. For example, implants with a high profile have a smaller diameter, and therefore these are well-suited for a woman with a small breast width who desires large breasts with fullness at the superior aspect of the breast.

Dr. Naidu is an investigator in the Breast Implant Follow-up Study (BIFS), a large-scale, post-approval study which supplements the data already accumulated on silicone breast implants. For more information on this study, please visit www.bifs.us.

Saline Breast Implants

Saline implants consist of a silicone shell that is filled with sterile salt water. Saline implants are less expensive than other types of implant. They also require a smaller incision than other implants because they are not filled until after being placed in the breast. One of the major benefits of saline breast implants is that, if the implant leaks or is damaged, your body easily absorbs the saline solution.

Some of our Brooklyn patients feel that saline breast implants do not look or feel as natural as other types of implants. They are also prone to rippling and wrinkling. Saline breast implants are approved for patients who at least 18 years old.

Silicone Breast Implants

Silicone breast implants are filled with silicone gel. Most of our Brooklyn patients believe that silicone breast implants look and feel much more like natural breast tissue. They are now available in both round and anatomic shapes. Silicone breast implants require a slightly larger incision because they are filled by the manufacturer prior to implantation.

Although silicone implants have a long history of safety, our Brooklyn patients must be monitored regularly by their physician to assess for rupture. Silicone breast implants are approved for patients who are at least 22 years old.

Gummy Bear Implants

Gummy bear implants are made of a thick cohesive gel, and are filled by the manufacturer prior to implantation in the operating room. These implants have a stable form that makes them feel like natural breasts, without the risk of leakage. Gummy bear implants are available in anatomical (natural) shapes and are a great choice for women who are slim, small-breasted, or lacking breast tissue due to a congenital issue or mastectomy.

Ideal Implants®

The Ideal Implant® has a series of silicone implant shells nested together and two separate chambers that hold saline. This unique design combines the natural look of silicone with the safety of saline. This simple internal structure is designed to control the movement of the saline and support the implant edges to reduce folding and wrinkling. In addition, the edges have been lowered to better fit the natural curve of the chest wall. Unlike silicone implants, leaks can be easily detected just by looking in the mirror. The Ideal Implant® is FDA-approved for cosmetic use in women aged 18 years or older.

What is the best position for breast implants?

The ideal position for breast implants depends mainly on the size of the implants and your anatomy. Generally implants are placed under the breast tissue (a subglandular placement), under the pectoralis muscle (a submuscular placement), or beneath the muscle and breast tissue (a dual plane placement).

Placing implants under the breast tissue is not advised for women with thin breast tissue. This lack of breast tissue means that the implant edges may become more visible, the implant may show rippling, and/or the implant edges can be felt. Subglandular placement can result in a higher risk of capsular contracture (scar tissue formation). Finally, mammograms are more difficult to perform with implant placement beneath the breast tissue.

Dr. Naidu’s preference is to use a dual plane placement, which creates a pocket for the implant beneath the muscle and the breast tissue. This dual plane placement provides optimal coverage of the implant. It also avoids visible implant edges and rippling.

Although the submuscular or dual-plane placement of breast implants involves a slightly longer recovery for our Brooklyn patients, the advantages of this placement are significant. The edges of the implant will be well-concealed and not as easily felt. Mammograms are simpler to perform because the breast tissue is easier to compress when the implant is placed behind the muscle. Finally, there is a significantly lower risk of capsular contracture when the implant is located behind the pectoralis muscle. For these reasons, Dr. Naidu will likely select this placement for your implants.

What is the best shape for breast implants?

Dr. Naidu will help you determine the best shape for your implants. Both saline and silicone breast implants are made in round shapes. Dr. Naidu prefers to use round implants for most cosmetic breast augmentation patients unless you have a specific preference.

Silicone implants also come in a teardrop (anatomic) shape. Teardrop implants provide additional volume to the upper portion of the breast. They are most commonly used for reconstruction patients who have lost much of this tissue due to surgery, injury or congenital conditions.

What breast implant surface is best?

The surface of the implant can be smooth or textured. Smooth implants move easily and feel soft, while textured implants have a slightly bumpy surface. Dr. Naidu most frequently uses smooth, round implants for her Brooklyn breast augmentation patients.

The benefit of texture is that the implant will not move as much within the breast pocket, which is especially important for tear-drop implants. The downside of textured implants is a higher risk of visible wrinkling. Also, in the rare instance that teardrop implants shift in position, a secondary surgery may be required.

Brooklyn Breast Augmentation Surgery

During your consultation, Dr. Naidu will explain her surgical plan. You will know the location of your incisions, the type of implant, and how your implants will be positioned.

Breast augmentation is performed at an outpatient basis at an ambulatory surgery center. For your comfort, Dr. Naidu prefers using general anesthesia. The procedure takes between 45 minutes to one hour. After being carefully monitored following the surgery, you will be allowed to go home to Brooklyn the same day. You must have someone to drive you to Brooklyn and stay with you for the first 24 hours after your surgery.

Dr. Naidu commonly uses one of three incision placements. Her choice is largely dependent on your goals. These incisions are the infra-mammary incision (located at the crease beneath the breast), the peri-areolar incision (located at the border between the breast skin and the pigmented skin of the nipple-areolar complex) and the axillary incision (located within the armpit).

As much as possible, incisions are hidden in the natural folds of the breasts or armpit, or around the areola. Dr. Naidu provides patients with silicone scar cream to be used for four to six weeks following surgery. In addition, using vitamin E oil can help scars to remain soft and flat. Incisions will fade over time.

During surgery, the incision for placement of the implant may be made at the edge of the nipple-areolar complex on the breast (peri-areolar), within the armpit (transaxillary), or beneath the breast at the crease (inframammary). Dr. Naidu most frequently uses the infra-mammary incision as this incision provides excellent access to the breast tissue and tends to heal very well. The implants are placed either behind the pectoralis muscle on the chest (subpectoral), or behind the breast tissue (subglandular). Patients with very little native breast tissue generally will have their implants placed in the subpectoral position to provide a natural shape with camouflage of the implant edges.

Breast Augmentation Recovery in Brooklyn

Many of Dr. Naidu’s Brooklyn patients are very independent women who are reluctant to ask for help. However, some degree of post-surgical assistance will be required. We strongly recommend that you create a plan to take care of yourself and your family, so that you can focus on your recovery.

Immediately following surgery, small pieces of tape are placed over the incisions. Drains, pumps, and support bras are not used. Your chest and arms will feel tight. You will be able to return to normal activity sooner if you follow Dr. Naidu’s specific instructions about arm exercises which should begin the evening after your surgery. Dr. Naidu will also give you a prescription for pain management, as well as instructions about how to take care of your incisions.

If it makes you more comfortable, a soft bra without an underwire may be worn the day after your surgery. Light activity, including walking, is permitted and encouraged immediately following surgery.

Many of our Brooklyn patients report very little pain, although the experience of pain varies widely. You may also experience bruising, swelling, and tenderness in your breasts which should resolve in several days. Swelling may persist for up to three weeks following your augmentation.

You should expect to remain out of work for three or four days. Heavy exercise, lifting, and sexual activity should be avoided for two weeks following surgery. Most swelling will resolve within one month. You may return to chest exercises six weeks after surgery. Your breasts will achieve their final shape and size within six months.

Complications of Breast Augmentation

During your consultation, Dr. Naidu will explain the risks of breast augmentation so that her Brooklyn patients can make an informed decision about this surgery. As with other surgeries, the risks of breast augmentation include bleeding, infection and scarring. Specific risks of breast augmentation include capsular contracture (scar tissue around the implant), rippling of the implants, and implant rupture.

Complications from breast augmentation can be reduced by providing a thorough medical history and carefully following your pre-operative and post-operative instructions.
